Caching broken in dexy 0.5.5

Since changing how artifacts are processed, i.e. running initial setup for all docs first, then running subsequent steps for each document in turn, caching appears to be broken. Documents are not updating when they need to, and 'dexy reset' has to be used to clear the cache so they do update.
